    Is it a new regulation that we complete a full nutritional assessment anually?  I assess each resident quarterly in my progress notes, ie.. appetite, weight/changes, meds, labs, skin etc.. but it seems a consultant during a mock survey said it is a regulation for annual nutritional assessement.  What form do you all use if you do do one?

    Is anyone working with a PACE program?  What form do you use for Nutritional Adequacy?  Is there a government form?

    I struggle with this... can anyone share what they do for such residents? Obviously they are in declining health, so I don't see the point of writing a care plan about no weight loss, etc. Any tips would be appreciated, thanks, Nancy McMahon

    At one of our quarterly meetings, I think on the new MDS 3.0 and survey issues, the presenter gave a name and email of someone from the state to ask questions.  Do you have this information?  A nearby facility is just ending their survey and the facility Dietitian was told by the Survey team Dietitian that fruit juice can not be counted at all as fruit.  I always thought if 100% juice and Vit C fortified then one serving of juice per day could count as a serving of fruit.
     
    I appreciate any information you can pass on to me!
